The office is open Monday through Thursday from 7:00am until 4:00pm. We will
attempt to schedule your appointment as soon as possible. If you are experiencing
significant pain and do not feel like you can wait for a scheduled appointment,
please inform the staff. Every attempt will be made to see you the same day.
Due to the nature of this practice, we see emergency patients regularly. We make
every effort to remain on schedule, but occasionally there will be a slight wait.
Please be prompt for your appointment as a courtesy to our other patients. We
reserve the doctor’s time exclusively for you. If you must reschedule,
please give us 24 hrs. notice if possible.

Your insurance coverage is a contract between you and your insurance
company. You are responsible for the payment to this
office. Our office will file your insurance claim as a courtesy to
you. We will attempt to verify all coverage prior to treatment. If
this cannot be done, payment in full will be requested
at the time of treatment. We offer Care Credit plans for our patients.
We participate in Delta Dental insurance plan, but will collect
co-payment on other types of insurance. The co-payment
requested at the time
of treatment is based on our knowledge with previous insurance
claims.
We accept Mastercard, Visa, American Express,
and Discover. If you have any further questions regarding payment,
please contact

arrangements need to be arranged prior to your appointment
for treatment.
